YEAR 7 VACCINATIONS - WEDNESDAY, 03 APRIL 2024

Next week on Wednesday, 03 April 2024, Year 7 students will receive their vaccination for Diphtheria, Tetanus and Pertussis (dTpa), and Human Papillomavirus (HPV).

Parents/carers have been emailed information and are required to complete BOTH consent forms. Your student will not receive the vaccination if both forms have not been submitted.

Form 1 - MGHS form to consent (emailed to parents/carers)
